Damage Claim Form

A damage claim form has a short clock attached. Most carrier liability regimes require notice within days, so the form's job is to capture a valid notification fast.

For hauliers, couriers, freight forwarders and fleet operators.

Notice periods are why this form should be short

Carrier liability conventions and standard terms set tight windows for reporting damage - sometimes three days, sometimes seven. A claimant who spends two weeks assembling a perfect claim has often lost the right to make it.

So this template collects a valid notification quickly: consignment reference, delivery date, what's damaged, and whether it was noted on the delivery receipt. Supporting documents follow. It also asks whether the packaging was damaged, which is the fact that most often decides liability.

The same facts, every time

When each report is written differently, patterns are invisible. Consistent fields mean you can look across a month and see the thing that keeps happening, rather than a folder of prose.

Dimensions, weight and dates - or it isn't a quote

A shipping enquiry without dimensions, weight and both postcodes cannot be priced, so it becomes a phone call. On a form, those are four fields and a dropdown.

The same applies to bookings and claims: the specifics are the entire message, and asking for them in a fixed order gives you something a dispatcher can act on immediately.

Easy enough to actually be used

A reporting process nobody follows produces no data. This one is a link that opens instantly on a phone and takes a couple of minutes, which is the only version that gets used on a busy day.

Edit it like a document, publish like a link

Add questions, remove questions, change the order and the wording, set your accent colour. When you save, the form is live at its own address - share it by link, QR code or a button on your site, and read the answers in your dashboard.

Questions

What's the most important field?

Whether the damage was noted on the delivery receipt at the time. A clean signature makes a concealed-damage claim much harder, and knowing that on day one changes how the claim is handled.

Can claimants attach photos?

Not yet - ask for a link and tell them what to photograph: the packaging before opening, the item, the label and the delivery note. Those four cover most disputes.

Can reports be submitted anonymously?

Yes, if you make the name optional. Nothing about the submitter is recorded otherwise - no IP address, no account, no tracking.

Can this go to more than one person?

Yes. Notification recipients are a list, so a report can reach a manager and a safety lead at the same time.

Can I export everything for an audit?

Yes - one click gives you a CSV of every response with its timestamp, which is usually exactly what an auditor or insurer asks for.

Can drivers submit forms from a phone?

Yes - that's the main way these get used. One small page, works on a poor signal, no app to install on anyone's personal phone.
