A driver application form is a compliance document. Licence categories, endorsements, digital tacho card, CPC status and medical - all of it decides whether someone can legally drive for you.

Endorsements and CPC, before the interview
The reason a promising driver can't start is almost never their driving - it's an expired CPC, a missing tacho card, or endorsements your insurer won't accept. Every one of those is a known fact that could have been asked on day one.
So this template asks for licence categories, points and endorsement codes, CPC hours and card expiry dates. Because responses export as a spreadsheet, you also get a register showing whose qualification card lapses next month - which is exactly what an audit asks for.

The same applies to bookings and claims: the specifics are the entire message, and asking for them in a fixed order gives you something a dispatcher can act on immediately.

Questions
Can applicants upload licence documents?
Not yet. Ask for the details and check the licence itself at interview - and use a licence checking service for ongoing verification.
Is it legal to ask about endorsements?
Yes - it's necessary for insurance and compliance. Ask for the codes and dates rather than an explanation, and be clear about which your insurer accepts.
Can applicants attach a CV or document?
Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.
Can I close applications on a deadline?
Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.
Can more than one person review them?
Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.
Can drivers submit forms from a phone?
Yes - that's the main way these get used. One small page, works on a poor signal, no app to install on anyone's personal phone.
