Volunteer Waiver Form
A volunteer waiver form protects the volunteer as much as the organisation. It records what they're being asked to do, that they know the risks, and who to call if something happens.

Volunteers occupy an odd legal space: not employees, but people you owe a duty of care. Most waiver forms are copied from employment paperwork and end up implying an employment relationship nobody intended.

It also asks about health only where it affects the tasks - which is proportionate and much better received.

Questions
Does a waiver stop a volunteer claiming if they're injured?
No, and it shouldn't try to. You still owe a duty of care and your insurance still applies. What it does is record that risks were explained - and set expectations about what the role involves.
Should volunteers under 18 use the same form?
Add parent or guardian consent and be more specific about supervision. Many organisations run a separate, shorter form for young volunteers.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
