Beneficiary Application Form
A beneficiary application form is filled in by someone asking for help, often at a bad moment. It has to establish eligibility without making the process feel like an interrogation.

The person completing this may be applying for a food parcel, a grant, or emergency support. Forms in this situation routinely read like fraud screening, and applicants abandon them - which means the help doesn't reach the people it was raised for.
So this template asks the eligibility questions plainly and explains why each is needed, offers "prefer not to say" where an answer isn't essential, and keeps the free-text questions few. It also asks how someone would prefer to be contacted, because not everyone can take a phone call in private.

Questions
How much evidence should we ask for up front?
As little as possible to make a decision. Ask what evidence they have rather than requiring documents with the application - people in crisis often can't produce paperwork quickly.
Should applications be anonymous?
They can't usually be, since you need to deliver the support - but say clearly who sees the application and that it isn't shared. That reassurance increases completion more than any other sentence on the page.
Can applicants attach a CV or document?
Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.
Can I close applications on a deadline?
Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.
Can more than one person review them?
Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.
Do you have a nonprofit discount?
The price is $10 a month for everything, which is usually below the discounted tier elsewhere. The free plan is genuinely usable for a single form.
