Sunday School Registration Form
A Sunday school registration form is a safeguarding record as much as a sign-up: who the child is, who may collect them, what the allergies are, and which permissions a parent has actually given.

The register by the door has to be right
On a Sunday morning the person on the door needs three things instantly: is this child registered, who is allowed to take them home, and is there an allergy. Nothing else matters in that moment.
So those three are required here and everything else is optional. Export the responses and you have the sheet that lives in the folder by the door - accurate, dated, and the same for every child rather than a mix of paper slips and remembered conversations.
One list, not five threads
Registrations that arrive by email, text and word of mouth become a headcount nobody trusts. You find out on the day that two people thought they were signed up and one wasn't.

Questions
How do we handle a child brought by a grandparent?
The authorised-collection field is where that lives. Ask parents to name everyone who might bring or collect, and to tell you when it changes.
Do we need this for a child who only visits occasionally?
Most churches use a shorter visitor version for one-offs - duplicate the template and cut it to name, contact number and allergies.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?

Can we route sensitive requests to one person only?
