Church Event Registration Form
A church event registration form answers the two questions every event team asks: how many chairs, and how many need feeding. It also asks the ones that get forgotten - children's ages, transport, accessibility.

Count the whole household, not the person filling it in
One person registers for four. Ask "will you attend?" and you'll be forty chairs short - so this template asks for numbers of adults and children separately, plus the children's ages, because that decides whether you need a crèche and how many volunteers.
Transport is the other question worth asking. Knowing who needs a lift turns a vague good intention into a rota you can actually organise, and it's the single biggest barrier to attendance for older members.
Know who's coming, and what they need
A name is the easy half. The awkward half - dietary needs, accessibility, emergency contacts, sizes - is the half that ruins a day when it's missing, and the half nobody thinks to send you.
So the form asks. Once, at registration, when the person actually knows the answer.
Volunteers, prayer requests and pastoral care
Most of what a church collects is sensitive in a way ordinary business forms aren't: a prayer request, a pastoral concern, a safeguarding detail. The form has to be easy for a 76-year-old on a phone and private enough to be trusted.

Questions
Can we take registrations for several events at once?
Ask which event in a dropdown and filter afterwards, or duplicate the form so each event has its own link and its own list. Duplicating is one click.
What if someone doesn't use email?
Print the form's QR code for the noticeboard, and let a volunteer fill it in on someone's behalf on the door. It works on any phone with no app.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
