Pastoral Care Request Form
A pastoral care request form is for the person who can't say it out loud at the door on a Sunday. Short, private, and clear about who will read it and how quickly they will reply.

Say who reads it, before anything else
Nobody asks for pastoral help through a form that might land in a shared office inbox. The single most important thing this page does is state plainly who receives it, and that it goes nowhere else.
The second is urgency. A form that can't distinguish "I'd like a coffee and a chat some time" from "I need someone today" will fail somebody. This template makes urgency a required question and says clearly, on the form, that it is not for emergencies - with the number to call instead.
Better than publishing your email address
A mailto link gets you spam and messages with no context. A form gets you the same enquiry with the details you need, in a list you can search, and no address for a scraper to harvest.
Volunteers, prayer requests and pastoral care
Most of what a church collects is sensitive in a way ordinary business forms aren't: a prayer request, a pastoral concern, a safeguarding detail. The form has to be easy for a 76-year-old on a phone and private enough to be trusted.
These pages carry no trackers and no cookie banner, so nothing is watching a person who came to ask for help. Responses go to whoever you nominate and nowhere else.

Who can see these requests?
Only the people you invite into your ava42 account, and only the addresses you set as notification recipients - so a pastoral form can go to one named person rather than a general inbox.
Should we allow anonymous requests?
Make the name optional if you want to, but ask for one way to reply. A request you can't respond to is worse than none, so the template asks for either an email or a phone number.
Will I get spam?
Far less than a published email address. Every form carries a hidden trap field that automated submitters fill in and humans never see, and those submissions are silently discarded.
Can I send messages to a shared inbox?
Yes. Notifications go to a list of addresses you choose, so a team alias works as well as one person.
Can I add this to my website?
Link to it from anywhere - a button, a nav item, a footer. It's a normal page with its own URL, so no embedding or code is required.
