Baptism Registration Form
A baptism registration form gathers what the register needs and what the service needs: the candidate's full name and date of birth, the parents, the godparents or sponsors, and a preferred date.

Spelling, and the names that go in the register
A baptism produces a permanent record and a certificate. Both need names spelled exactly right, in full, including the ones nobody uses day to day - and a name taken down in a corridor conversation is a name that will be wrong on the certificate.
Asking families to type it themselves solves that at the source. This template also asks about the preparation class and whether either parent has been baptised, which are the questions clergy otherwise have to raise awkwardly at a first meeting.
Registration that closes itself
The worst version of this is taking registrations you can't honour. Close the form when you're full and everyone with the link sees that immediately, instead of signing up and finding out later.
Volunteers, prayer requests and pastoral care
Most of what a church collects is sensitive in a way ordinary business forms aren't: a prayer request, a pastoral concern, a safeguarding detail. The form has to be easy for a 76-year-old on a phone and private enough to be trusted.

Questions
Can we use this for adult baptisms too?
Yes - the candidate fields work for any age, and the parent questions can be left blank. Some churches duplicate the form and cut it down for adults.
Can families request a specific date?
Ask for a first and second preference, as the template does. It saves a round of emails, and you keep the final say.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?

Can we route sensitive requests to one person only?
