Sponsorship Application Form
A sponsorship application form works in both directions: it lets a company tell you what they want from the relationship, and it lets you find out whether their budget and their brand fit before anyone drafts an agreement.
For associations, professional bodies and membership organisations.
Sponsorship dies in the details
The deal is agreed in a meeting and then stalls for a month over logo files, who signs off the copy, whether they get the attendee list, and what "headline" actually includes. All of it was knowable on day one.
This template asks for the tier, the budget range, the marketing contact and what they need in return, in writing. It also asks the question that saves the most time: is there anything your organisation can't be associated with?
The same questions, in the same order, every time
Applications that arrive as emails and attachments are impossible to compare - one has the dates, another has the budget, none of them have both. You end up rebuilding each one by hand before you can make a decision.
A single form fixes that at the source: every application has the same fields, so reviewing is reading a table rather than reconstructing one.
Membership admin is mostly chasing
Renewals, address changes, committee nominations and event places all arrive as replies to a newsletter nobody was tracking. The work isn't the decision - it's assembling a list that's already out of date.
A form per task gives you one list per task, in the member's own words, with the same fields every time. Export it and your register update becomes a paste rather than an afternoon.
One link, and a written record
Share one link and every application lands in one place, timestamped, in the applicant's own words - which is exactly what you want if a decision is ever questioned.
Yours to change, immediately
Nothing here is fixed. Rewrite a question, reorder them by dragging, add your brand colour, delete half of it. Save and the live form updates - there's no separate publish step and no deploy to wait for.

Questions
Should I publish our sponsorship tiers on the form?
Yes - put the tiers and what each includes in the introduction, then ask which they want. Companies self-select far more accurately than they do in conversation.
Can we use this for in-kind sponsorship?
The template includes it as an option. Ask what they'd provide and its approximate value, so you can compare it against a cash tier.
Can applicants attach a CV or document?
Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.
Can I close applications on a deadline?
Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.
Can more than one person review them?
Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.
Can members update their own details?
Yes - publish the form and send the link with your usual mailing. Each submission is a dated record of what the member told you, which is exactly what you want when a register is questioned.
