The badge, the lunch and the lift
The three things attendees remember are their name spelled correctly, whether there was food they could eat, and whether they could get into the room. All three are decided at registration, and all three are usually asked too late.
So this template asks for the badge name separately from the legal name, makes dietary requirements a real question rather than an afterthought, and asks about access needs in a way people can answer without a phone call. Can I take payment for tickets?
Not through the form. Most organisers collect registrations here and invoice, or send a payment link in the confirmation email - which also lets you check capacity before charging anyone.
