Chapter Registration Form

A chapter registration form is how a national body keeps track of what its local groups actually are: who runs them, where they cover, how many members they have, and how to reach them when something needs saying.

For associations, professional bodies and membership organisations.

Chapters change officers every year

The single biggest source of stale data in a federated organisation is the committee that changed in March and told nobody. Then a mailing goes to a chair who stepped down, and a chapter misses a deadline it never heard about.

This template is built to be re-sent annually: the same fields, so this year's answers sit next to last year's, with the date each was submitted. Registering a new chapter and re-confirming an existing one use the same form and the same list.

Registration that closes itself

The worst version of this is taking registrations you can't honour. Close the form when you're full and everyone with the link sees that immediately, instead of signing up and finding out later.

Membership admin is mostly chasing

Renewals, address changes, committee nominations and event places all arrive as replies to a newsletter nobody was tracking. The work isn't the decision - it's assembling a list that's already out of date.

A form per task gives you one list per task, in the member's own words, with the same fields every time. Export it and your register update becomes a paste rather than an afternoon.

Works from a phone, in a queue

Half of these get filled in on a phone while someone is standing in front of you. One small page, no app, no login - it opens and it submits, even on a bad signal.

One minute to make it your own

Every label, option and placeholder on this template is editable, and you can add whatever this template doesn't ask. Save to publish, share the link, and export the responses whenever you need them in a spreadsheet.

Questions

Can we use this for annual re-registration too?

That's the main use. Send the link each year, ask for the current officers, and the responses give you a dated history of who ran each chapter when.

Can chapters see each other's details?

No. Only people you invite into your ava42 account can see responses, so chapter submissions are private to the national office.

Can I stop registrations when we're full?

Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.

Can I collect one registration per person or per group?

Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.

Can I get the list as a spreadsheet?

Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.

Can members update their own details?

Yes - publish the form and send the link with your usual mailing. Each submission is a dated record of what the member told you, which is exactly what you want when a register is questioned.
