Continuing Education Registration Form
A continuing education registration form has one job ordinary event forms don't: it has to capture the details that let you issue a credit certificate afterwards - licence number, jurisdiction, and the exact course attended.

The certificate is the product
People attend CPD because they need the credit. If your records don't tie a named licence holder to a specific session on a specific date, you cannot issue the certificate, and the whole event was decoration.

Questions
Can I issue certificates from this?
Not automatically - but the export gives you a clean spreadsheet of names, licence numbers and courses attended, which is exactly what a mail-merge or a certificate tool needs.
What if credits differ by jurisdiction?
Ask for the jurisdiction and use a conditional question to ask anything extra that only applies there. Hidden questions stop being required, so nobody is asked for a number their state doesn't issue.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
