Membership Application Form
A membership application form has to establish eligibility before it establishes enthusiasm. Category, qualifications, experience and - where you need them - proposers, asked in one pass so the membership committee can decide from one document.
For associations, professional bodies and membership organisations.
Eligibility first, everything else after
The most common failure in membership admin is a warm application that turns out not to qualify. By then someone has invested a week, and saying no is much harder than it should have been.
So the eligibility questions come first here, and the category is chosen explicitly rather than assumed. Applicants who don't qualify find out from the form's own wording, and the committee reads only applications it can actually approve.
The same questions, in the same order, every time
Applications that arrive as emails and attachments are impossible to compare - one has the dates, another has the budget, none of them have both. You end up rebuilding each one by hand before you can make a decision.
A single form fixes that at the source: every application has the same fields, so reviewing is reading a table rather than reconstructing one.
Membership admin is mostly chasing
Renewals, address changes, committee nominations and event places all arrive as replies to a newsletter nobody was tracking. The work isn't the decision - it's assembling a list that's already out of date.
A form per task gives you one list per task, in the member's own words, with the same fields every time. Export it and your register update becomes a paste rather than an afternoon.
One link, and a written record
Share one link and every application lands in one place, timestamped, in the applicant's own words - which is exactly what you want if a decision is ever questioned.
Yours to change, immediately
Nothing here is fixed. Rewrite a question, reorder them by dragging, add your brand colour, delete half of it. Save and the live form updates - there's no separate publish step and no deploy to wait for.

Questions
Can we take the membership fee here?
Not on the form. Most bodies approve first and then invoice or send a payment link - which is usually the right order anyway, because you don't want to refund a declined application.
Can we ask for proposers or references?
Yes - the template asks for two, with an email each. Because they're structured fields, chasing a reference is a copy-and-paste rather than a hunt through the application.
Can applicants attach a CV or document?
Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.
Can I close applications on a deadline?
Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.
Can more than one person review them?
Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.
Can members update their own details?
Yes - publish the form and send the link with your usual mailing. Each submission is a dated record of what the member told you, which is exactly what you want when a register is questioned.
