Roofing Quote Form
A roofing quote form is mostly about access and area. What the roof is made of, how steep it is, how high, and whether scaffolding can go up - those decide the price before a tile is counted.

Scaffolding is a line item, not a detail
On many roofing jobs the access costs more than the materials. A two-storey rear elevation over a conservatory, or a terrace with no side access, can add more to a quote than the entire tiling.
So this template asks about storeys, obstructions and whether scaffolding can be erected from the ground, alongside the roof covering and pitch. Ask those on the form and your first number is close; ask them on site and you've already spent a morning on a job you may not win.
A quote request that actually contains the job
"How much do you charge?" is unanswerable, and answering it with a range makes you look expensive to some and cheap to others. The fix isn't a longer reply - it's a better question.
Scope, timing, location and budget are on this form because those four answers turn a vague enquiry into something you can price with confidence, usually without a call.
Site access, scope and dates decide the price
Two identical-sounding jobs price differently because of parking, access, floor level and whether anyone lives there during the work. Those are the questions that stop a quote becoming an argument.
Asking on a form also gives you a written scope. When the job changes - and it will - you have what was originally described, dated, in the customer's own words.

Questions
Should I offer a repair or replacement option?
Ask which they think they need, with a "not sure" option - it's often the most honest answer, and it opens the conversation about whether a patch is throwing money away.
How do I handle insurance work?
Add a question asking whether they're claiming, as this template does. Insurance jobs need different paperwork and a different timeline, and knowing up front saves a misunderstanding.
Can I ask for photos or files?
Not yet - uploads are on the roadmap. In the meantime, most people ask for a link to photos, or reply to the response with an email asking for them. Everything else about the job is captured here.
Can I see where each quote request came from?
Yes. Add a hidden tracking parameter to the link (for example one link on your site and another on your Google profile) and it's stored with the response.
