Electrical Service Request Form
An electrical service request form has to sort emergencies from installations in the first question. A tripping circuit tonight and a rewire next month need completely different responses.

Urgency and the consumer unit tell you what van to send
The two facts that shape an electrical job are how urgent it is and what's already on the wall. An old fuse board with rewireable fuses means a different conversation from a modern board with RCBOs - sometimes it means the job can't be done as asked.

How do I stop everything being marked urgent?
Describe what each level means next to the options - "no power to the property" versus "one socket not working". People self-select accurately when the definitions are visible.
Can I collect certificate details for compliance work?
Ask for what you need to plan the job here, and issue certificates through your usual process. Responses export as a spreadsheet if you need a record of what was requested.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?
Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.
Will I know straight away?
