Plumbing Service Request Form
A plumbing service request form has one job before all others: find out whether water is currently escaping. Everything else on the page is secondary to that question.

Ask whether the stopcock is off
The most useful question you can put on a plumbing form isn't what's broken - it's whether they've turned the water off. It tells you the severity, it tells you whether this is tonight or Thursday, and it prompts a customer who hadn't thought of it to go and do it.
The rest of the form is triage: what kind of fixture, how old the system is, whether there's a combi or a tank. Those decide the parts you bring, which decides whether the job is done in one visit.
Fewer no-shows, fewer surprises
No-shows and "I thought I booked the other thing" both come from the same place: a booking nobody wrote down properly. When the customer types it themselves, there's a record, in their words, of exactly what was asked for.
Site access, scope and dates decide the price
Two identical-sounding jobs price differently because of parking, access, floor level and whether anyone lives there during the work. Those are the questions that stop a quote becoming an argument.
Asking on a form also gives you a written scope. When the job changes - and it will - you have what was originally described, dated, in the customer's own words.

Questions
Can I use this for out-of-hours calls?
Have the form tell people to phone for genuine emergencies, and keep it for everything else. A form is the wrong channel for an active leak at 2am, and saying so protects your reputation.
Should I ask about the boiler on a plumbing form?
Ask what kind of system they have - combi, system, or tank in the loft. It changes what you can diagnose remotely and what parts are worth carrying.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?
Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.
Will I know straight away?
