Change Order Request Form

A change order request form is the difference between a variation and an argument. What changed, who asked for it, what it costs, what it does to the programme, and a signature - captured on the day, not reconstructed at the end.

Every disputed invoice is an undocumented variation

Verbal changes on site are normal and unavoidable. What causes the fight three months later is that nobody wrote down what was agreed, so the client remembers a small adjustment and the contractor remembers two extra days.

This template makes writing it down take ninety seconds on a phone. It asks who requested the change, what the cost and programme impact are, and requires an explicit approval tick - so the record exists before the work does, with a timestamp nobody can argue with.

Asking on a form also gives you a written scope. When the job changes - and it will - you have what was originally described, dated, in the customer's own words.

Questions

Can the client approve it on the same form?

Most contractors send the link to the client so they submit the approval themselves - that's a stronger record than a tick box you filled in on their behalf.

Can I number change orders?

Add a reference field and use your own sequence. Every response is also dated and listed in order, so you have a chronology whether you number them or not.

Can reports be submitted anonymously?

Yes, if you make the name optional. Nothing about the submitter is recorded otherwise - no IP address, no account, no tracking.

Can this go to more than one person?

Yes. Notification recipients are a list, so a report can reach a manager and a safety lead at the same time.

Can I export everything for an audit?
