A recruitment intake form is the briefing a recruiter needs before sourcing anyone. What the role actually does, what's genuinely essential, and what the process is.

Separate the essential from the wish list, on the record

Every job brief contains fifteen requirements, of which four are real. Sourcing against all fifteen finds nobody; sourcing against a guess gets candidates rejected for a reason that was never stated.

So this template forces the split: three genuine must-haves, then everything else as desirable. It also asks for the interview process and the decision-maker's availability - because the most common reason a good candidate is lost is a hiring manager who couldn't interview for two weeks.

What a good intake form saves you

The first message from a new candidate almost never contains what you need. You reply asking one question; they answer a different one; a week later you still can't start. Every round trip is a day, and the candidates who go quiet are usually the ones who got tired of answering.

One form ends that. Ask everything once, get a complete picture, and start the same day. The questions in this template are the ones you end up asking anyway - the form just gets them asked up front.

The slow part of hiring isn't reading CVs - it's normalising them. Availability, location, right to work, salary expectation and notice period asked identically make a shortlist a sort rather than a project.

Questions

Why ask for the interview process up front?

Because it's the biggest predictor of whether you'll fill the role. Four stages over five weeks loses candidates to a two-stage competitor, and the hiring manager should hear that on day one.

Should I insist on a salary band?

Yes - a role advertised without one attracts fewer applicants and wastes time on candidates priced out of it. Push back on "competitive".

Can I add or remove questions?

Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.

Where do the answers go?

Into your dashboard, where you can read, filter and export them as a spreadsheet. You can also have every response emailed to you the moment it arrives, or as a daily or weekly digest.

Do I need a website?

Can candidates attach a CV?

Not yet - uploads are on the roadmap. Asking for a link to a CV or profile works today, and the structured answers are usually what you screen on anyway.
