Staffing Request Form

A staffing request form is a temp booking. How many people, doing what, on which shifts, starting when - and who they report to when they arrive.

For recruiters, staffing agencies and in-house talent teams.

A site contact and a start time, or nobody knows where to go

Temp placements fail on arrival. Workers turn up at a gate with no name to ask for, at a time nobody expected, without the PPE the site requires - and the client concludes the agency is unreliable.

So this template asks for the site contact and mobile number, the exact start time, where to report, and what PPE is needed. Those four answers are the difference between a placement that works and a refund.

Take orders while you're working

You can't answer the phone with your hands full, and the person calling won't wait. A form takes the order at 11pm on a Sunday and has it waiting when you open.

Comparable candidates, not forty differently-formatted CVs

The slow part of hiring isn't reading CVs - it's normalising them. Availability, location, right to work, salary expectation and notice period asked identically make a shortlist a sort rather than a project.

It's also a better candidate experience: a five-minute form that clearly asks what matters beats an inbox that never replies.

One place to check what's due

Every order lands in one list, timestamped, filterable and exportable. No scrolling back through a chat thread to find out what Tuesday looks like.

Make it yours in a minute

Change the wording, drop the questions you don't need, add the ones you do, put your own colour on it. Saving publishes it, and you get a link to paste into your website, your email signature or a message. Responses land in your dashboard and download as a spreadsheet whenever you want them.

Questions

How much notice do you need?

State your own minimum on the form. Same-day requests are possible for some roles and impossible for others, and being clear stops disappointment.

Should I ask about pay rates?

Ask what the client expects to pay, and be clear about your charge rate separately. Ambiguity between pay rate and charge rate is the biggest source of billing disputes in staffing.

Can I take payment on the form?

Not yet. Most people use this to capture the order and then invoice or take payment on collection - which also means you can confirm availability before charging anyone.

Can I limit what people can order?

Yes. Options are lists you control, so only what you actually sell appears - and you can close the form when you're at capacity.

Will I be told about new orders straight away?

Yes, if you turn on live notifications: each order is emailed to you as it arrives. Otherwise they collect in your dashboard.

Can candidates attach a CV?

Not yet - uploads are on the roadmap. Asking for a link to a CV or profile works today, and the structured answers are usually what you screen on anyway.
