Temporary Worker Application Form

A temporary worker application form is registered once and used for months, so it has to capture availability precisely and everything a payroll needs.

For recruiters, staffing agencies and in-house talent teams.

Availability and transport, because that's how shifts get filled

Filling a 6am shift in an industrial estate with no bus route is a transport problem, not a skills problem. A register that records tickets but not whether someone drives is a register that can't fill half its bookings.

So this template asks about transport and how far someone will travel, alongside a day and shift grid. Combined with tickets and right to work, a consultant can filter to the eleven people who could actually do Thursday nights in that postcode.

Screen before you spend time

Most of the work in an application process is on the ones that were never eligible. Asking the disqualifying questions first - dates, requirements, basic facts - means you spend your reading time on the real candidates.

Comparable candidates, not forty differently-formatted CVs

The slow part of hiring isn't reading CVs - it's normalising them. Availability, location, right to work, salary expectation and notice period asked identically make a shortlist a sort rather than a project.

It's also a better candidate experience: a five-minute form that clearly asks what matters beats an inbox that never replies.

Long enough to decide, short enough to finish

Applications fail in two directions: too short to judge, or so long that good candidates give up. This template asks what a decision genuinely needs, split into steps so it never looks like a wall.

One minute to make it your own

Questions

How often should the register be refreshed?

Ask people to tell you when availability changes, and re-send the link every few months. A stale register is worse than a small one because consultants stop trusting it.

What payroll details should we collect here?

Enough to set someone up - payment method preference, whether they're PAYE or umbrella - but not bank details on a web form. Collect those through your payroll system.

Can applicants attach a CV or document?

Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.

Can I close applications on a deadline?

Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.

Can more than one person review them?

Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.

Can candidates attach a CV?

Not yet - uploads are on the roadmap. Asking for a link to a CV or profile works today, and the structured answers are usually what you screen on anyway.
