Background Check Consent Form
A background check consent form has to say exactly which checks will be run. "We may carry out checks" is not informed consent, and in several jurisdictions it isn't lawful either.
For recruiters, staffing agencies and in-house talent teams.
List each check, because consent has to be specific
Screening covers wildly different things: identity, right to work, employment references, education, credit, criminal records, driving licence, social media. A candidate may be entirely comfortable with five of those and surprised by the sixth.
So this template lists each check separately with a plain description, and asks the candidate to acknowledge them. It also tells them they'll see any adverse result before a decision is made - which is a legal requirement in many places and simply decent everywhere.
Say what they're agreeing to, in plain words
A consent form that nobody reads protects nobody. The statements in this template are short, specific and separate - so ticking one is a real decision rather than a formality.
Edit the wording to match what you actually do. It should sound like your practice, not like a legal template.
Comparable candidates, not forty differently-formatted CVs
The slow part of hiring isn't reading CVs - it's normalising them. Availability, location, right to work, salary expectation and notice period asked identically make a shortlist a sort rather than a project.
It's also a better candidate experience: a five-minute form that clearly asks what matters beats an inbox that never replies.
Digital, dated, and searchable
Every response is stored with the date and time it was submitted, and the whole set exports as a spreadsheet. Finding one person's answers takes seconds rather than a filing cabinet.

Questions
Should candidates upload identity documents here?
No. Collect the consent and the details a screening provider needs, and verify documents through a channel built for it. A web form is the wrong place for a passport scan.
Can we ask about criminal convictions?
Only where the role legally permits it, and the rules on spent and filtered convictions vary considerably. Ask consent to run a check rather than asking the candidate to self-disclose, and take advice.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.
Can candidates attach a CV?
Not yet - uploads are on the roadmap. Asking for a link to a CV or profile works today, and the structured answers are usually what you screen on anyway.
