Legal Document Request Form

A legal document request form handles the steady stream of people asking for a copy of a will, a deed, a file or a court document - and its job is establishing whether they're entitled to it.

For solicitors, attorneys, paralegals and small firms.

Entitlement, before the search

Producing a document for someone not entitled to it is a professional problem, and the request rarely arrives with the evidence. A niece asking for a copy of a will, a former employee asking for a file, a buyer's solicitor asking for a deed - each requires different authority.

So this template asks who the requester is and what entitles them, with conditional questions per document type. It also asks whether a fee has been agreed, because searching an archive and copying a file is real work that often gets done for free by accident.

The same facts, every time

When each report is written differently, patterns are invisible. Consistent fields mean you can look across a month and see the thing that keeps happening, rather than a folder of prose.

Conflict checks and the facts, before the first meeting

The initial consultation is expensive time. Names of other parties, dates, what has already happened - collected in advance, they let you run a conflict check and arrive knowing the matter.

A written intake is also a cleaner file note: what the client said at the outset, dated, unedited, in their own words.

Easy enough to actually be used

A reporting process nobody follows produces no data. This one is a link that opens instantly on a phone and takes a couple of minutes, which is the only version that gets used on a busy day.

One minute to make it your own

Every label, option and placeholder on this template is editable, and you can add whatever this template doesn't ask. Save to publish, share the link, and export the responses whenever you need them in a spreadsheet.

Questions

Can requesters upload proof of entitlement?

Not yet. Ask what evidence they hold - grant of probate, power of attorney, written authority - and verify it through your usual channel before releasing anything.

Should we charge for document retrieval?

If you do, say so on the form with the amount. Archive retrieval and copying costs are legitimate and much easier to raise before the work than after.

Can reports be submitted anonymously?

Yes, if you make the name optional. Nothing about the submitter is recorded otherwise - no IP address, no account, no tracking.

Can this go to more than one person?

Yes. Notification recipients are a list, so a report can reach a manager and a safety lead at the same time.

Can I export everything for an audit?

Yes - one click gives you a CSV of every response with its timestamp, which is usually exactly what an auditor or insurer asks for.

Is a web form confidential enough for a legal enquiry?

Pages are served over encrypted connections and responses stay in your account. Even so, say plainly on the form that submitting it does not create a lawyer-client relationship, and ask people not to send privileged detail until you've engaged.
