Questions
Can this satisfy client due diligence requirements?
It collects the information; it does not verify it. Identity documents and source-of-funds evidence must go through your own compliant process - never as attachments to a web form.
Should we ask about source of funds?
Ask the category, as this template does, and follow up properly. Knowing early that funds come from a gift or an overseas account changes what evidence you'll need.
Can I add or remove questions?
Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.
Where do the answers go?

Is a web form confidential enough for a legal enquiry?
