Law Firm Consultation Form
A law firm consultation form books the first meeting and does the conflict check at the same time. Both need the names of the other parties, so ask once.

Conflict checking is the reason this form exists
A consultation that has to be abandoned because the firm already acts for the other side wastes an hour of fee-earner time and puts the firm in an awkward position. It's entirely preventable: you need the names before the meeting, not at it.
So the other parties are a required field here, ahead of the appointment preferences. The form also asks how the client heard about you and whether they've had advice already - which tells you whether this is a first opinion or a second, and those meetings run very differently.

Questions
Should the first consultation be free?
Whatever you decide, say it on the form with the duration. Ambiguity about whether a first meeting is chargeable is a common complaint and an easy one to avoid.
Can we do the conflict check automatically?
Not from the form - it collects the names into a consistent field, and your own system does the check. That's still a large improvement on names buried in prose.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?
Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.
Will I know straight away?

Is a web form confidential enough for a legal enquiry?
