Client Consent Form
A client consent form in a legal practice records what the client has agreed to: the scope of the instruction, who may be spoken to, how fees work, and how they want to be contacted.
For solicitors, attorneys, paralegals and small firms.
Consent to contact is a professional obligation, not a courtesy
The complaints that arise most often in practice are not about the law - they're about communication. A client who didn't want their spouse spoken to, or who never agreed to email, or who says they were never told about the hourly rate.
Each of those is a one-line question. This template asks them separately and stores the answers with a timestamp, so the file contains what was agreed at the outset rather than someone's recollection of the first meeting.
Say what they're agreeing to, in plain words
A consent form that nobody reads protects nobody. The statements in this template are short, specific and separate - so ticking one is a real decision rather than a formality.
Edit the wording to match what you actually do. It should sound like your practice, not like a legal template.
Conflict checks and the facts, before the first meeting
The initial consultation is expensive time. Names of other parties, dates, what has already happened - collected in advance, they let you run a conflict check and arrive knowing the matter.
A written intake is also a cleaner file note: what the client said at the outset, dated, unedited, in their own words.

Questions
Does this replace a client care letter?
No - it supplements it. Your engagement letter still sets out terms; this records the client's specific consents and preferences in a form you can search and export.
Can we use it for conflict checks?
Ask for the names of other parties, as this template does, and run the check through your own process. The form gives you the names in a consistent field rather than buried in an email.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.
Is a web form confidential enough for a legal enquiry?
Pages are served over encrypted connections and responses stay in your account. Even so, say plainly on the form that submitting it does not create a lawyer-client relationship, and ask people not to send privileged detail until you've engaged.
