Family Law Intake Form
A family law intake form is filled in by someone having a difficult year. It has to gather what you need without reading like an interrogation, and it has to ask about safety.

Ask about safety, and say what you'll do with the answer
A proportion of family enquiries involve domestic abuse, and those need a different response - immediately, and sometimes without a call to a shared home number. A form that doesn't ask will find out three weeks later.
So this template asks whether there are safety concerns and whether it's safe to call, and it explains why it's asking. It also asks whether the other party knows about the enquiry - which is the question that determines how you make contact and is almost never on a standard intake form.
Why ask it all up front
Half-finished information is worse than none: it looks like you can begin, and then you can't. A client who fills in one page has told you enough to quote, schedule and prepare - without a single back-and-forth.
It also sets the tone. A form that asks precisely, and only once, reads as a business that has done this before. That impression is doing real work while your competitor is still emailing "quick question".
Conflict checks and the facts, before the first meeting
The initial consultation is expensive time. Names of other parties, dates, what has already happened - collected in advance, they let you run a conflict check and arrive knowing the matter.
A written intake is also a cleaner file note: what the client said at the outset, dated, unedited, in their own words.
It works on a phone, on a bad signal

Questions
Is a web form appropriate for family matters?
For a first contact, yes - many people find it easier than a phone call. Ask about safe contact methods, and never send detail to an email address the other party may access.
Should we ask about finances at intake?
Ranges and broad categories only. Full financial disclosure is a formal process; an intake form just needs to know whether there's a house, a business or a pension in scope.
Can I add or remove questions?

Is a web form confidential enough for a legal enquiry?
