Financial Planning Intake Form
A financial planning intake form is the household picture: who's in it, what comes in, what's committed, and what matters most in the next five years. It's the information a plan is built from, collected once and in writing.

Plan for a household, not a person
Almost every real decision - pensions, protection, school fees, moving house - involves someone else. A form that only asks about one person produces a plan that falls apart at the first conversation at home.
So this template asks about the household: partner, dependants, whose income is whose, and what is already committed each month. It also asks people to rank their priorities, which is the question that most often reveals a plan is being built for the wrong goal.
Why ask it all up front
Half-finished information is worse than none: it looks like you can begin, and then you can't. A client who fills in one page has told you enough to quote, schedule and prepare - without a single back-and-forth.
It also sets the tone. A form that asks precisely, and only once, reads as a business that has done this before. That impression is doing real work while your competitor is still emailing "quick question".
What accountants ask that other trades don't
Entity type, year end and VAT status decide the work, the deadlines and the fee - and none of them turn up in the first email. Asking early means you quote once, properly, instead of revising after you discover there are two companies and a partnership.
Money questions also carry a duty of care. Nothing on this form is shared, tracked or sent anywhere except your own account, and the page carries no third-party scripts at all - which is a shorter answer than most software can give a client who asks.

Questions
Why ask people to rank priorities?
Because everything is important until it's ordered. Dragging five priorities into order takes ten seconds and tells you more about what a client actually wants than a paragraph would.
Can I split this over more than one page?
