Financial Advice Consultation Form

A financial advice consultation form does the groundwork a first meeting usually burns: what they're trying to achieve, by when, what they already have in place, and how they feel about risk. An hour of fact-finding becomes ten minutes of reading.

For accountants, bookkeepers and small practices.

The first meeting should start at the second question

Fact-finding in the room is expensive and unreliable - people don't have their pension details to hand, and they under-report what they already hold. Sent in advance, the same questions get answered at a desk with the paperwork nearby.

Asking about goals and timeframe in writing also changes the conversation. People are more candid on a form about wanting to retire early or being nervous about losing money, and that candour is exactly what shapes good advice.

Why ask it all up front

Half-finished information is worse than none: it looks like you can begin, and then you can't. A client who fills in one page has told you enough to quote, schedule and prepare - without a single back-and-forth.

It also sets the tone. A form that asks precisely, and only once, reads as a business that has done this before. That impression is doing real work while your competitor is still emailing "quick question".

What accountants ask that other trades don't

Entity type, year end and VAT status decide the work, the deadlines and the fee - and none of them turn up in the first email. Asking early means you quote once, properly, instead of revising after you discover there are two companies and a partnership.

Money questions also carry a duty of care. Nothing on this form is shared, tracked or sent anywhere except your own account, and the page carries no third-party scripts at all - which is a shorter answer than most software can give a client who asks.

It works on a phone, on a bad signal

People fill these in from a car park, a waiting room, a sofa at 11pm. This form is one small page that opens the instant they tap the link and submits without needing anything clever in their browser - so an answer sent on two bars still reaches you.

Edit it like a document, publish like a link

Add questions, remove questions, change the order and the wording, set your accent colour. When you save, the form is live at its own address - share it by link, QR code or a button on your site, and read the answers in your dashboard.

Questions

Isn't this a regulated fact-find?

No - treat it as a pre-meeting enquiry, not a suitability assessment. It gathers context so the first conversation is useful; your regulated fact-find and suitability process still happen in your own compliant process.

Should I ask for account numbers or values?

Ask for ranges rather than exact values and don't ask for account numbers. Ranges are enough to prepare for a first meeting, and they keep sensitive detail out of a web form.

Can I add or remove questions?

Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.

Where do the answers go?

Into your dashboard, where you can read, filter and export them as a spreadsheet. You can also have every response emailed to you the moment it arrives, or as a daily or weekly digest.

Do I need a website?

No. Every form gets its own link you can paste into an email signature, a text message, a QR code or a social profile. If you do have a site, you can link to it from anywhere on it.

Is it suitable for client financial data?

For contact details, entity information and a description of the work, yes - responses are stored in your account and never shared. Don't use a web form for bank credentials or full account numbers; ask for those over a channel built for it.
