Budget Request Form

Approvals are slow because requests are inconsistent
Requests that arrive as emails all describe different things. One has a quote attached and no cost centre; the next has a business case and no amount. The approver's first job becomes finding out what is actually being asked for.
This template fixes the shape of a request: the same fields, in the same order, with the amount and the date as required. What used to be three emails of clarification becomes a decision on first read - and every request stays on file, dated, whether it was approved or not.
The same questions, in the same order, every time
Applications that arrive as emails and attachments are impossible to compare - one has the dates, another has the budget, none of them have both. You end up rebuilding each one by hand before you can make a decision.
A single form fixes that at the source: every application has the same fields, so reviewing is reading a table rather than reconstructing one.
What accountants ask that other trades don't
Entity type, year end and VAT status decide the work, the deadlines and the fee - and none of them turn up in the first email. Asking early means you quote once, properly, instead of revising after you discover there are two companies and a partnership.
Money questions also carry a duty of care. Nothing on this form is shared, tracked or sent anywhere except your own account, and the page carries no third-party scripts at all - which is a shorter answer than most software can give a client who asks.

Questions
Can this route to different approvers by amount?
Not automatically - it collects requests and notifies the people you choose. Most teams add a threshold question to the form and let the finance approver escalate; you can also run separate forms for capital and operating spend.
Can staff see other people's requests?
No. Responses are only visible to people you invite into your ava42 account, so requesters see nothing but their own confirmation.
Can applicants attach a CV or document?
Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.
Can I close applications on a deadline?
Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.
Can more than one person review them?
Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.
Is it suitable for client financial data?
For contact details, entity information and a description of the work, yes - responses are stored in your account and never shared. Don't use a web form for bank credentials or full account numbers; ask for those over a channel built for it.
