Volume is the price, so ask for volume
Bookkeeping is priced by transactions, bank accounts and mess - not by the size of the company. Two businesses with the same turnover can be an hour a month apart, and you cannot tell which is which from an email that says "looking for a bookkeeper".
This template asks for the numbers that actually drive the work, plus the question everyone forgets: how many months are outstanding. Catch-up work is a separate job, and finding out about it after you've quoted a monthly rate is a painful conversation.
A quote request that actually contains the job
"How much do you charge?" is unanswerable, and answering it with a range makes you look expensive to some and cheap to others. The fix isn't a longer reply - it's a better question.
Scope, timing, location and budget are on this form because those four answers turn a vague enquiry into something you can price with confidence, usually without a call.
What accountants ask that other trades don't
Entity type, year end and VAT status decide the work, the deadlines and the fee - and none of them turn up in the first email. Asking early means you quote once, properly, instead of revising after you discover there are two companies and a partnership.
Money questions also carry a duty of care. Nothing on this form is shared, tracked or sent anywhere except your own account, and the page carries no third-party scripts at all - which is a shorter answer than most software can give a client who asks.
Let them show you, not describe it
A sentence about the problem is a guess; a photo is a fact. This template asks for the details in the client's own words *and* keeps the questions short enough that they answer all of them.

Questions
Should I quote from the form, or still have a call?
Most bookkeepers send an indicative monthly range from the form and use the call to confirm it. The point isn't to remove the conversation, it's to stop the conversation being about basic facts.
What if they don't know their transaction volume?
The template offers ranges rather than a number, which almost everyone can answer. A range is enough to price a bookkeeping engagement within about 10%.
Can I ask for photos or files?
Not yet - uploads are on the roadmap. In the meantime, most people ask for a link to photos, or reply to the response with an email asking for them. Everything else about the job is captured here.
Can I see where each quote request came from?
Yes. Add a hidden tracking parameter to the link (for example one link on your site and another on your Google profile) and it's stored with the response.
