Credit Application Form

A credit application form is how you decide whether to let a customer buy now and pay later. Business details, the limit they want, trade references and a clear acceptance of your terms - collected in one pass so the decision doesn't take a week of emails.

For accountants, bookkeepers and small practices.

References are the part everyone leaves out

Trade references are the single most useful thing on a credit application and the field most likely to be skipped. Make them required and ask for two, with a contact email each, and you can check a customer in an afternoon instead of taking a view and hoping.

The other half is the terms. When the acceptance is a separate, specific tick box next to the payment terms - rather than buried in a paragraph - you have a dated record of what was agreed, which is what you'll want if an account goes bad.

Screen before you spend time

Most of the work in an application process is on the ones that were never eligible. Asking the disqualifying questions first - dates, requirements, basic facts - means you spend your reading time on the real candidates.

What accountants ask that other trades don't

Entity type, year end and VAT status decide the work, the deadlines and the fee - and none of them turn up in the first email. Asking early means you quote once, properly, instead of revising after you discover there are two companies and a partnership.

Money questions also carry a duty of care. Nothing on this form is shared, tracked or sent anywhere except your own account, and the page carries no third-party scripts at all - which is a shorter answer than most software can give a client who asks.

Long enough to decide, short enough to finish

Applications fail in two directions: too short to judge, or so long that good candidates give up. This template asks what a decision genuinely needs, split into steps so it never looks like a wall.

Make it yours in a minute

Change the wording, drop the questions you don't need, add the ones you do, put your own colour on it. Saving publishes it, and you get a link to paste into your website, your email signature or a message. Responses land in your dashboard and download as a spreadsheet whenever you want them.

Questions

Can I ask for bank details or a director's date of birth?

You can, but think hard about it. A web form is fine for business identifiers and references; for anything you'd use to verify identity or take payment, use a channel built for it and reference it from the form.

Can I set a different form for higher limits?

Yes - duplicate the template and ask for more (accounts, guarantees, directors) on the version you send to customers requesting a larger limit. Each copy has its own link.

Can applicants attach a CV or document?

Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.

Can I close applications on a deadline?

Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.

Can more than one person review them?

Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.

Is it suitable for client financial data?

For contact details, entity information and a description of the work, yes - responses are stored in your account and never shared. Don't use a web form for bank credentials or full account numbers; ask for those over a channel built for it.
