Why this one asks about entity type

Sole trader, partnership and limited company lead to genuinely different work, different deadlines and different fees. Asking early means you can quote properly instead of revising a quote later.

The template uses that answer: choose Limited company and it asks for the company number; choose anything else and that question never appears. Nobody is asked for a number they don't have, which is the difference between a form that feels thorough and one that feels like paperwork.

What a good intake form saves you

The first message from a new client almost never contains what you need. You reply asking one question; they answer a different one; a week later you still can't start. Every round trip is a day, and the clients who go quiet are usually the ones who got tired of answering.

One form ends that. Ask everything once, get a complete picture, and start the same day. The questions in this template are the ones you end up asking anyway - the form just gets them asked up front.

What accountants ask that other trades don't

Entity type, year end and VAT status decide the work, the deadlines and the fee - and none of them turn up in the first email. Asking early means you quote once, properly, instead of revising after you discover there are two companies and a partnership.

Can I use this to onboard clients I've already agreed to take on?

What if a client doesn't know their year end?

Leave the question optional, as the template does. It's better to get the form finished with a gap you can fill from Companies House than to have someone abandon it looking for a date.
