Workshop Registration Form
A workshop registration form should tell you two things before the day: how mixed the room will be, and what each person is hoping to walk out with. Both change how you run it.

Experience level decides where you start
A workshop pitched at the average of a mixed room bores the experienced and loses the beginners. Knowing the spread in advance lets you adjust - a pre-read for some, a faster opening, a breakout that groups people usefully.
Asking what each person wants to leave with does something else: it makes them prepare. People who have written down an objective participate more, and you get to open by naming what the room actually came for.
Registration that closes itself
The worst version of this is taking registrations you can't honour. Close the form when you're full and everyone with the link sees that immediately, instead of signing up and finding out later.
Qualify before you give away an hour
A free discovery call with the wrong person costs the same as one with the right person. Goals, timeline and budget on the form mean the calls you take are with people ready to work with you.

Questions
Should I ask for objectives if I won't use them all?
Ask anyway - reading twenty objectives takes five minutes and reliably changes one thing about your plan. Quoting two of them in the opening also lands well.
Can I cap numbers?
Close the form when you reach capacity and everyone with the link sees a closed page instead of the questions. Reopen it if someone drops out.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?
Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.
Won't asking about budget put people off?
