Life Coaching Intake Form
A life coaching intake form starts wider than a business one, so it needs structure: a quick rating across the areas of someone's life, then depth on the one or two that brought them here.

Rate the areas, then talk about one
Asked "what would you like to work on?", most people write something general about being happier. Asked to rate eight areas of their life out of ten, they produce a map - and the two low scores are usually not the ones they mentioned first.
That's why this template uses quick scale questions before the open ones. It takes a client thirty seconds, and it gives you the thing an interview rarely surfaces: which part of their life they've stopped mentioning.
What a good intake form saves you
The first message from a new client almost never contains what you need. You reply asking one question; they answer a different one; a week later you still can't start. Every round trip is a day, and the clients who go quiet are usually the ones who got tired of answering.
One form ends that. Ask everything once, get a complete picture, and start the same day. The questions in this template are the ones you end up asking anyway - the form just gets them asked up front.
Qualify before you give away an hour
A free discovery call with the wrong person costs the same as one with the right person. Goals, timeline and budget on the form mean the calls you take are with people ready to work with you.
It changes the first conversation, too: instead of twenty minutes of background, you open with what they wrote and spend the time on the actual problem.
Fast enough that people finish it
Abandonment is mostly a speed problem. A form that takes a second to appear and submits in one request gets finished; one that loads a framework first does not. This is a single self-contained page - no scripts to download, nothing to wait for.

Questions
Isn't a wheel-of-life exercise better done live?
Doing it in advance means you spend the session on what the scores mean rather than collecting them. Many coaches re-run it at three months to show movement.
What if someone discloses something clinical?
Have a referral route ready and say on the form that coaching isn't therapy. This template asks whether they're working with a professional already, which is the question that usually surfaces it.
Can I add or remove questions?
Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.
Where do the answers go?
Into your dashboard, where you can read, filter and export them as a spreadsheet. You can also have every response emailed to you the moment it arrives, or as a daily or weekly digest.
Do I need a website?

Won't asking about budget put people off?
