Group Coaching Registration Form

A group coaching registration form does something one-to-one intake doesn't: it assembles a group. Who they are, what they want, and whether they'll actually show up each week determines whether the cohort works for everybody in it.

One wrong person changes the whole group

Group coaching lives and dies on composition. Someone at a completely different stage, or unwilling to speak, or unable to make two thirds of the sessions, changes the experience for everyone else - and you can't fix it in week four.

So this template asks about stage and sector so you can balance the cohort, and asks bluntly about attendance and willingness to contribute. It also asks what they'd rather not discuss in front of others, which is the question that prevents the most awkward session you'll ever run.

The worst version of this is taking registrations you can't honour. Close the form when you're full and everyone with the link sees that immediately, instead of signing up and finding out later.

Qualify before you give away an hour

A free discovery call with the wrong person costs the same as one with the right person. Goals, timeline and budget on the form mean the calls you take are with people ready to work with you.

Questions

Can I run a waiting list with this?

Yes - add a cohort option for "join the waiting list", or close the form when a cohort fills and reopen it for the next one.

Should I ask people to commit to dates?

Ask which dates they'd miss. It's more honest than asking for a commitment, and it tells you whether the schedule works before you publish it.

Can I stop registrations when we're full?

Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.

Can I collect one registration per person or per group?

Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.

Can I get the list as a spreadsheet?

Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.

Won't asking about budget put people off?

It puts off the people who were never going to book, which is the point. Make it a range rather than a number and it reads as professional rather than transactional.
