A video production inquiry form has to establish scope, and in video scope means days. Filming days, edit days, and whether a script exists.

Is there a script? The answer changes the project by weeks
"We need a two-minute video" can mean a one-day shoot from a finished script, or a three-week project that starts with working out what the video should say. Both arrive as the same enquiry.
So this template asks about the script and the storyboard explicitly, and asks who the on-screen talent is. A video needing a professional presenter, a voiceover artist and licensed music is a substantially different quote from one shot with the founder in their office.
A quote request that actually contains the job
"How much do you charge?" is unanswerable, and answering it with a range makes you look expensive to some and cheap to others. The fix isn't a longer reply - it's a better question.
Scope, timing, location and budget are on this form because those four answers turn a vague enquiry into something you can price with confidence, usually without a call.
Date, location, deliverables - the three that set the price
Every shoot enquiry needs the date, where it is, and what they actually need at the end. Without those you can't say whether you're free, let alone what it costs.
Asking on a form also surfaces the things that quietly double a job: travel, a second shooter, usage rights, a same-week turnaround.
Let them show you, not describe it
A sentence about the problem is a guess; a photo is a fact. This template asks for the details in the client's own words *and* keeps the questions short enough that they answer all of them.

Questions
What length should I ask for?
Ask for the target duration and where it'll be used - a 30-second social cut and a three-minute website film need different shooting. Length drives edit time more than shoot time.
Should I ask about music and licensing?
Yes. Licensed music, stock footage and voiceover are real costs, and the client usually hasn't thought about them. Better to raise it in the quote than the invoice.
Can I ask for photos or files?
Not yet - uploads are on the roadmap. In the meantime, most people ask for a link to photos, or reply to the response with an email asking for them. Everything else about the job is captured here.
Can I see where each quote request came from?
Yes. Add a hidden tracking parameter to the link (for example one link on your site and another on your Google profile) and it's stored with the response.
