Vehicle Service Request Form

A vehicle service request form is the routine one, which is exactly why it should be self-service. Registration, mileage, which service level, and whether the MOT is due at the same time - booked without a single phone call.

For garages, dealerships, body shops and mobile mechanics.

Bundle the MOT while you have the car

The most valuable question on a service booking form is when the MOT is due. Half of customers don't know, and the ones who do would rather do both in one visit than come back in six weeks.

Asking it here fills your test bay from bookings you already had, and saves the customer a second morning without a car. The template also asks about known problems separately from the service itself, so an advisory doesn't arrive as a surprise on collection.

Fewer no-shows, fewer surprises

No-shows and "I thought I booked the other thing" both come from the same place: a booking nobody wrote down properly. When the customer types it themselves, there's a record, in their words, of exactly what was asked for.

Registration, mileage, symptoms - before it arrives

A car you know nothing about is an hour of diagnosis you didn't book. Registration, mileage and a plain description of the noise let you plan parts and time before the keys are on the counter.

It also stops the two most expensive misunderstandings in the trade: the job that was bigger than the quote, and the courtesy car nobody arranged.

Confirm in writing, immediately

The moment they submit, they see a confirmation and you get an email. No wondering whether it arrived - which is the thing that makes people phone you anyway.

Make it yours in a minute

Change the wording, drop the questions you don't need, add the ones you do, put your own colour on it. Saving publishes it, and you get a link to paste into your website, your email signature or a message. Responses land in your dashboard and download as a spreadsheet whenever you want them.

Questions

How do I explain the difference between service levels?

Put a line in the form's introduction describing what interim, full and major include. People choose accurately when told, and it removes the most common call you get.

Can customers pick a courtesy car or collection?

Yes - both are tick boxes here. Knowing in advance is what makes them possible to offer at all.

Does this book the slot automatically?

No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.

Can I limit the options people can pick?

Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.

Will I know straight away?

Yes, if you want to be. Turn on live notifications and every request is emailed to you the moment it arrives; otherwise collect them and read them in your dashboard.

Can I ask for a registration or VIN?

Yes - they're ordinary text questions, and asking for both is common. What you can't do yet is look a registration up automatically; the form collects what the customer types.
