Remoteness is the risk nobody prices in
A sprained ankle at home is inconvenient. The same ankle four hours from a road, in a country where the nearest hospital is a flight away, is an evacuation - and evacuation without insurance is one of the fastest ways to bankrupt a family.
So this template states the remoteness plainly and requires confirmation of insurance including repatriation. It also asks for a next-of-kin contact who is *not* travelling, which is the field that matters most and the one most often filled in with a fellow traveller.

A consent form that nobody reads protects nobody. The statements in this template are short, specific and separate - so ticking one is a real decision rather than a formality.

Questions
Does a waiver limit our liability?
Partially at best, and rarely for negligence - it varies by jurisdiction. Its real value is evidencing that risks were explained and that insurance and medical information were collected.
Should we require proof of insurance?
For anything remote or adventurous, yes - ask for the insurer and policy number, and check it covers the activities and repatriation. A policy that excludes trekking above 2,000m is no policy on a mountain.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
