Undeclared conditions are why claims are refused
The most common reason a travel claim fails is a pre-existing condition that wasn't declared - usually because the traveller didn't think a managed condition counted, or hoped a cheaper premium would cover it.
So this template asks directly and explains plainly that non-disclosure means a refused claim. It also asks about activities - skiing, diving, riding, anything with a motor - because those are excluded by default on most standard policies and travellers assume they're included.

Questions
How much medical detail do we need?
Enough to refer to a medical screening: conditions, whether they're stable, and any recent treatment or investigations. The insurer's own screening does the detail.
What activities should we ask about?
Anything excluded by default on a standard policy - winter sports, diving, riding, motorcycling, trekking above a certain altitude, competitive sport. Most travellers don't know these need adding.
Can I ask for photos or files?
Not yet - uploads are on the roadmap. In the meantime, most people ask for a link to photos, or reply to the response with an email asking for them. Everything else about the job is captured here.
Can I see where each quote request came from?
Yes. Add a hidden tracking parameter to the link (for example one link on your site and another on your Google profile) and it's stored with the response.
