An excursion booking form needs to check that the party can actually do the excursion. Fitness, mobility and age limits matter more than the date.

Fitness level, honestly described
"Moderate walking" means an easy stroll to one person and a hike to another. Someone who books a canyon walk expecting a coach tour ruins their day and slows the group, and nobody was misled - the words were just too vague.
So this template describes the demands concretely - hours on foot, terrain, steps, altitude - and asks the party to confirm they're comfortable with it. It also asks about mobility and age limits, which are the two things that legitimately prevent a booking.
Requests you can actually schedule
A name and "sometime next week" isn't a booking. What you need is when they can come, what they want, and how to reach them if the slot moves.

How should I describe difficulty?
In numbers, not adjectives: hours walking, metres of ascent, whether there are steps or uneven ground. People can judge that; "moderate" they cannot.
Should I ask about swimming ability?
For anything on or in water, always - and ask specifically rather than assuming. It's a safety question and it takes one dropdown.
