SEO Client Intake Form
An SEO client intake form has to uncover the site's history before anyone touches it. Past penalties, a migration, bought links - all of it constrains what's possible.
For agencies, studios and marketing teams.
Ask about past link building, gently but directly
A site that bought links in 2019 has a problem no amount of good content will fix, and the client usually doesn't remember it as anything sinister - just "an agency that did some marketing".
So this template asks about previous SEO work, migrations and any traffic drop the client noticed. Those three questions surface most historical damage before you set expectations, which is the difference between a realistic plan and a promise you can't keep.
Why ask it all up front
Half-finished information is worse than none: it looks like you can begin, and then you can't. A client who fills in one page has told you enough to quote, schedule and prepare - without a single back-and-forth.
It also sets the tone. A form that asks precisely, and only once, reads as a business that has done this before. That impression is doing real work while your competitor is still emailing "quick question".
A brief that doesn't need a briefing
Clients describe outcomes; you need inputs. Objective, audience, deadline, budget, who signs it off - asking for those in a fixed order turns a vague request into a scope you can staff.
It's also the fastest way to spot the project that isn't ready: no budget, no deadline, four decision-makers. Better to know before the kick-off call.
It works on a phone, on a bad signal
People fill these in from a car park, a waiting room, a sofa at 11pm. This form is one small page that opens the instant they tap the link and submits without needing anything clever in their browser - so an answer sent on two bars still reaches you.
Make it yours in a minute
Change the wording, drop the questions you don't need, add the ones you do, put your own colour on it. Saving publishes it, and you get a link to paste into your website, your email signature or a message. Responses land in your dashboard and download as a spreadsheet whenever you want them.

Questions
Should I ask for keyword targets at intake?
Ask what they *think* they should rank for - it's revealing, and often wrong in a useful way. Real targets come from research, but their assumptions tell you about their market.
What access matters most?
Search Console and Analytics, then the CMS. Without Search Console you're working blind on the one dataset that actually shows what Google sees.
Can I add or remove questions?
Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.
Where do the answers go?
Into your dashboard, where you can read, filter and export them as a spreadsheet. You can also have every response emailed to you the moment it arrives, or as a daily or weekly digest.
Do I need a website?
No. Every form gets its own link you can paste into an email signature, a text message, a QR code or a social profile. If you do have a site, you can link to it from anywhere on it.
Can I use a different form per service?
Yes - duplicate the template and edit each copy, so a paid-media enquiry asks different questions from a brand project. Each has its own link and its own list of responses.
