Marketing Client Intake Form

A marketing client intake form is the difference between month one being productive and month one being a scavenger hunt for logins and brand files.

For agencies, studios and marketing teams.

Ask for access on day one, because chasing it costs a month

Every agency has lost the first four weeks of a retainer waiting for analytics access, ad account permissions, CMS logins and brand files. The work can't start, the client thinks nothing is happening, and the relationship begins badly.

So this template asks for every access requirement explicitly, as a checklist, alongside objectives and measurement. It also asks who the single decision-maker is - because a retainer approved by committee is a retainer where nothing ships.

What a good intake form saves you

The first message from a new client almost never contains what you need. You reply asking one question; they answer a different one; a week later you still can't start. Every round trip is a day, and the clients who go quiet are usually the ones who got tired of answering.

One form ends that. Ask everything once, get a complete picture, and start the same day. The questions in this template are the ones you end up asking anyway - the form just gets them asked up front.

A brief that doesn't need a briefing

Clients describe outcomes; you need inputs. Objective, audience, deadline, budget, who signs it off - asking for those in a fixed order turns a vague request into a scope you can staff.

It's also the fastest way to spot the project that isn't ready: no budget, no deadline, four decision-makers. Better to know before the kick-off call.

Fast enough that people finish it

Abandonment is mostly a speed problem. A form that takes a second to appear and submits in one request gets finished; one that loads a framework first does not. This is a single self-contained page - no scripts to download, nothing to wait for.

One minute to make it your own

Every label, option and placeholder on this template is editable, and you can add whatever this template doesn't ask. Save to publish, share the link, and export the responses whenever you need them in a spreadsheet.

Questions

Should I ask about previous agencies?

Yes, and ask what didn't work. It's the most useful question on the form - it tells you the client's expectations, their process, and the trap you're about to walk into.

How do I ask for access without it looking like homework?

Frame it as "what we'll need to get started" and list it as checkboxes. Clients tick what they can provide and flag what they can't, which is exactly what you need to know.

Can I add or remove questions?

Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.

Where do the answers go?

Into your dashboard, where you can read, filter and export them as a spreadsheet. You can also have every response emailed to you the moment it arrives, or as a daily or weekly digest.

Do I need a website?

No. Every form gets its own link you can paste into an email signature, a text message, a QR code or a social profile. If you do have a site, you can link to it from anywhere on it.

Can I use a different form per service?

Yes - duplicate the template and edit each copy, so a paid-media enquiry asks different questions from a brand project. Each has its own link and its own list of responses.
