Every salon loses bookings to the simple fact that service and phone calls happen at the same time. The client who couldn't get through doesn't call back - they book somewhere that answered.
A request form collects it all while you work: what they want, which stylist, when they can come, and - crucially - whether they're new. New clients need longer and often a consultation, and that's the single question that stops a diary going wrong.

The questions that protect a colour appointment - previous dye, home kits, allergies, patch test dates - are exactly the ones that get skipped when a booking happens in a DM. Asked on a form, they're answered honestly and in writing.
It also protects the diary. Knowing the current colour and the goal before the appointment is the difference between a two-hour slot and a five-hour rescue.

Can clients choose their stylist?
Yes - add your team as a dropdown with "first available" as the first option. It also shows you which stylists are in demand.
Can this replace my booking system?
It replaces the phone for requests, not a live diary. You confirm each request, which is the right way round when appointments overrun.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?
Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.
Will I know straight away?
Yes, if you want to be. Turn on live notifications and every request is emailed to you the moment it arrives; otherwise collect them and read them in your dashboard.
Can I use this for patch test records?
Yes, and many salons do: the response is stored with the date it was submitted, which is the record you want if a reaction is ever disputed. Have your insurer confirm what they expect you to keep.
