Is a tick box enough, or do I need a signature?
In many places a clear, dated, affirmative action is accepted - but it varies, and your insurer may have a view. Check with them, and if they want a wet signature, use this as the pre-appointment record and sign a printout.
Should I list every possible risk?
List the ones that actually happen and the ones that are serious. A wall of text reads as boilerplate; five specific sentences read as a warning.
Is this legal advice?
