A hair consultation form is a conversation you can have before the appointment: what their hair actually does, what they've been told before, how much time they'll spend on it, and what they want to change.

Questions
Isn't this what the consultation is for?
The consultation is for looking at their hair and agreeing a plan. Collecting the background in advance means you spend that time on the plan rather than the background.
Can clients send inspiration photos?
Yes - add a file question and they can attach up to three, 20 MB each. A link to a saved post also works, which is where most people keep them anyway.
Can I add or remove questions?
Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.
Where do the answers go?
Into your dashboard, where you can read, filter and export them as a spreadsheet. You can also have every response emailed to you the moment it arrives, or as a daily or weekly digest.
Do I need a website?
No. Every form gets its own link you can paste into an email signature, a text message, a QR code or a social profile. If you do have a site, you can link to it from anywhere on it.
Can I use this for patch test records?
Yes, and many salons do: the response is stored with the date it was submitted, which is the record you want if a reaction is ever disputed. Have your insurer confirm what they expect you to keep.
