A booking two hours away at 6am is a different job from one down the road at midday, and both look identical in an email that says "are you free on the 14th?". You need location and ready-by time in the first message.

Questions
Should I charge for travel, and ask about it here?
Ask for the postcode or town and quote travel separately. It's much easier to explain a travel charge in a quote than to discover the venue is 90 minutes away after agreeing a price.
Can I use this for bridal as well as events?
You can, but bridal usually deserves its own form - the trial, the party size and the timings need more space. Duplicate the template and adjust.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?

Can I use this for patch test records?
