Pet Sitting Booking Form

A pet sitting booking form is a handover document. Everything the sitter needs to run someone else's household, written down while the owner is calm rather than packing.

For groomers, dog walkers, boarding kennels, sitters and vets.

Write the routine down while nobody is in a hurry

Pet sitting instructions given verbally at the door, twenty minutes before a taxi, are the ones that go wrong. Which food, how much, which door stays shut, where the spare litter is, whether the cat is allowed out.

So this template collects the whole routine in writing, days before. It also collects the vet's details and explicit authorisation for emergency treatment - the one thing a sitter cannot improvise at 8pm on a Sunday with an owner on a plane.

Stop booking by phone tag

Half your owners would rather book at 10pm than call at 10am, and the ones who do call reach a voicemail while you're working. Every missed call is a booking that went somewhere open.

A request form takes bookings while you're busy, asleep or driving - with the details you need written down rather than remembered.

The animal's details, not just the owner's

Breed, age, temperament, vaccinations, vet contact, whether they can be with other dogs - these decide whether you can take a booking at all, and they never arrive in the first message.

Asked on a form, you get them once, in writing, for every animal - including the awkward ones about biting and anxiety that owners will admit on a form and downplay on a call.

It's a request, not a calendar takeover

You stay in control: requests arrive, you confirm the ones that work. No syncing, no double-booking, nothing to install - just the details you need in the order you need them.

Edit it like a document, publish like a link

Add questions, remove questions, change the order and the wording, set your accent colour. When you save, the form is live at its own address - share it by link, QR code or a button on your site, and read the answers in your dashboard.

Questions

What's the most important field?

Emergency vet authorisation, with a spending limit. Without it a sitter is stuck between delaying treatment and committing an owner to a bill they didn't agree to.

Should I ask for a second contact?

Always - someone local who can act if the owner is unreachable or abroad. It's the field sitters are most grateful for and owners most often forget.

Does this book the slot automatically?

No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.

Can I limit the options people can pick?

Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.

Will I know straight away?

Yes, if you want to be. Turn on live notifications and every request is emailed to you the moment it arrives; otherwise collect them and read them in your dashboard.

Can I ask for vaccination or vet details?

Yes - dates, vet name and practice phone number are ordinary questions here, and having them written down before the first visit is exactly what insurers expect.
