Recall determines whether the dog can be let off at all
The most important question on a dog walking form is whether the dog comes back when called - because an off-lead dog that doesn't is the single biggest liability a walker carries.
So this template asks about recall specifically, and asks the owner to authorise off-lead walking as a separate decision rather than assuming it. It also asks about lead reactivity, which decides whether the dog can be walked in a group or needs to go solo.

Questions
Should I ask for a key or use a key safe?
Ask how they'd like you to get in and have a written key policy. It's a trust question and a liability one, and putting it on the form starts that conversation properly.
Can I walk dogs from different households together?
Ask each owner's consent explicitly, as this template does. Most owners are happy, and the ones who aren't would rather say so on a form than discover it later.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?
