Pet Grooming Booking Form

A pet grooming booking form has to know the coat's condition, not just the breed. A well-maintained doodle and a matted one are two hours apart and sometimes a different job entirely.

For groomers, dog walkers, boarding kennels, sitters and vets.

Matting changes the job from a groom to a clip-off

A severely matted coat cannot be brushed out humanely, and the honest answer is often a short clip. Owners who weren't warned arrive expecting a haircut and leave upset, and the groomer is blamed for a situation that took six months to create.

So this template asks about matting and the date of the last groom directly, and warns that a heavily matted coat may need clipping short for the animal's welfare. That one sentence prevents most grooming complaints.

Stop booking by phone tag

Half your owners would rather book at 10pm than call at 10am, and the ones who do call reach a voicemail while you're working. Every missed call is a booking that went somewhere open.

A request form takes bookings while you're busy, asleep or driving - with the details you need written down rather than remembered.

The animal's details, not just the owner's

Breed, age, temperament, vaccinations, vet contact, whether they can be with other dogs - these decide whether you can take a booking at all, and they never arrive in the first message.

Asked on a form, you get them once, in writing, for every animal - including the awkward ones about biting and anxiety that owners will admit on a form and downplay on a call.

It's a request, not a calendar takeover

You stay in control: requests arrive, you confirm the ones that work. No syncing, no double-booking, nothing to install - just the details you need in the order you need them.

Edit it like a document, publish like a link

Add questions, remove questions, change the order and the wording, set your accent colour. When you save, the form is live at its own address - share it by link, QR code or a button on your site, and read the answers in your dashboard.

Questions

Can owners send photos of the coat?

Yes - a file question takes up to three photos at 20 MB each, which is the fastest way to judge a coat. The matting and last-groom questions get you most of the way to an accurate time estimate.

How do I handle aggressive animals?

Ask about handling directly - nail trims, ears, being held, muzzle tolerance. Most owners answer honestly on a form, and it's the difference between a safe appointment and an injury.

Does this book the slot automatically?

No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.

Can I limit the options people can pick?

Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.

Will I know straight away?

Yes, if you want to be. Turn on live notifications and every request is emailed to you the moment it arrives; otherwise collect them and read them in your dashboard.

Can I ask for vaccination or vet details?

Yes - dates, vet name and practice phone number are ordinary questions here, and having them written down before the first visit is exactly what insurers expect.
