Dog Daycare Registration Form

A dog daycare registration form is a risk assessment. Temperament with other dogs, vaccination status, neutering, and how the dog behaves when it's overwhelmed.

For groomers, dog walkers, boarding kennels, sitters and vets.

Ask about resource guarding and recall, not just temperament

"Is your dog friendly?" gets a yes from almost every owner, including the owners of dogs that guard toys, dislike being approached while eating, or can't be called off. In a room of twenty dogs those are the behaviours that cause incidents.

So this template asks specific behavioural questions rather than a general one, and asks what the dog does when it's had enough. Owners answer those honestly because they're practical questions, not a judgement on their dog.

Registration that closes itself

Questions

Should we require neutering?

State your own policy on the form. Many daycares require it over a certain age for safety reasons - and asking the question means you find out before the dog is at the door.

How do we handle a dog that fails assessment?

Ask for the behavioural detail up front so most mismatches are caught on the form. For the rest, a trial day with an honest conversation afterwards is far better than a refusal at drop-off.

Can I stop registrations when we're full?

Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.

Can I collect one registration per person or per group?

Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
